After April 29, 2019, we will no longer be associated with Diabetes360. Please understand that we intend to set up practice at a new location very soon and will keep you informed of those developments. You will receive another letter from us with specific information including address and phone number and a process for enrolling as our patient.

Diabetes360 will cease all clinical care at all locations at the end of business effective April 29, 2019. We want to assure you that your medical records will be safely and securely stored until you are ready to retrieve them. Any questions about your medical records should be directed to Desert River Solutions, the medical records custodian. They can also be reached by email at, phone at (480) 577-3150, or fax at (520) 214-0068.

As your physician we understand that each patient has unique needs. For each of you, the timing of your next visit with a diabetes provider will differ. Some of you will need careful follow-up soon and others may be comfortable to wait for weeks or months. Whatever the nature of your condition, it is important that you continue to get proper and timely care. You have the right to request to see any diabetes provider of your choosing, whether in Arlington, Las Colinas, Plano or elsewhere. You may transfer your care to any physician of choice or you may request a copy of your records be provided directly to you. Additionally, until you’ve chosen a new diabetes provider, for your medical care you may reach out to:

  • your primary care physician
  • the Dallas County Medical Society for a list of diabetes providers
  • your insurance carrier for an approved list of providers
  • your local urgent care clinic
  • or any medical crisis or exacerbations, please immediately go to the nearest Emergency Room.

Once we have established our new practice, you may arrange for the transfer of your medical records back to us once a new practice is established, or we will be able to request medical records at the new clinic during your first visit.

Our main goal is to ensure you receive the proper care during this transition period. If you have questions, please call our offices and our staff will try to answer any questions to assist in promoting your care.

Regardless of your choice for ongoing care, we thank you for allowing us to care for you in the past and to learn about each of you as individuals. We wish you good things in the future and want you to know that we will do whatever we can to minimize inconvenience to you.
